What to Expect at Your First Appointment
During your first visit to a provider, you will undergo a comprehensive evaluation by one or more of our trained chiropractors. Your chiropractor will ask you a series of questions, discuss the problems you're experiencing, and devise a plan of action going forward. In addition, the chiropractor may ask for information pertaining to the patient's lifestyle, activity and fitness level, and functional capabilities. You should also share relevant medical history that may affect your treatment plan.
What to Bring
The provider you are connected with will instruct you on which items you will need to bring on your first visit. Here are some of the things you may be asked to provide:
- Photo ID
- Insurance card
- Physician referral (if necessary)
- Applicable co-pay (if necessary)
- L&I claim form (if injury is work-related)
